I’m currently employed by a company on a UK salary of £45k.
I’m leaving them next month to move to another employer. However, my current employer would like me to do work for them on an ad-hoc basis (evenings and weekends).
I’m struggling to work out what I should charge them (both method and actual charge).

Sounds like you want to be charging them double what you currently earn (about 50% of costs are hidden from be employee, eg HR, finance and back office admin).
To be on track for a £90k income for the year you want to be looking at £400 per day.
It also depends what line of work you’re in and how many years experience you’ve got, so adjust accordingly.
